What is included
- Up to 20 booked episodes. For each one Rivendell finds the options, books the appointment, pays the provider on the plan-funded card and matches the receipt to the booking.
- A receipt for each episode: provider, cash price paid, the source and date of that price, and the appointment date.
- The side-by-side on your 20 claim lines: allowed amount, Medicare, posted cash and the price Rivendell confirmed, with totals, within 5 business days of receiving the lines.
What the plan provides
- 20 de-identified claim lines: CPT code, zip or NPI, allowed amount.
- A funding account or card program for the episodes.
- A signed business associate agreement, before any member data.
- One operations contact.
Timeline
- Kickoff call.
- First bookings within 10 business days of the kickoff call.
- The pilot runs for 60 days.
Exit
If the pilot ends before 20 episodes are booked, the unused part of the prepaid amount is refunded: $75 for every episode of the 20 not booked.
